Your First Visit

We look forward to meeting you on your first visit to our office. During this time, we will help you get acquainted with our office and learn about your orthodontic treatment. We understand that each patient’s orthodontic treatment is different, and we promise to give you the personal attention you deserve. Your first orthodontic visit will consist of an examination and discussion of potential treatment options. This important visit will address your orthodontic needs to determine if this is the proper time to begin treatment. All patients under 18 years of age need to be accompanied by a parent or guardian for their first visit.

After we have evaluated your records, we conduct a treatment consultation to explain all aspects of your treatment in detail. We will provide a complete clinical diagnosis regarding the nature of the problem, a written summary of the findings and a detailed plan that outlines your treatment, goals and estimated fees. We encourage questions from both the patient and the parent, and as such, we provide ample time to answer your questions.

It is our mission to provide you with affordable, high-quality treatment that fits your budget! We offer a variety of payment options to meet your needs. Please ask us about financing options, credit card payments, and bank drafts. We thoroughly explain your options, so that we can accommodate your needs.

Emergency Info

What to do in case of emergency
Call our office as soon as possible if you break or loosen any of your appliances. Please do not come directly to the office – by calling us, you will allow us to create a time to see you. Even if you have a regular appointment scheduled, call us immediately to notify us if you need an appliance repaired.

Loose brackets or bands
Call our office immediately for advice if a bracket or wire is loosened. The bracket may need to be re-fitted as soon as possible. You may have a situation that requires cutting a wire or sliding a bracket off a wire at night or over the weekend. If you need to cut a wire in case of emergency, you may use fingernail clippers that have been washed and sterilized in alcohol. Please call our office the next business day, so that we may schedule an appointment for you.

Wire irritations
Sometimes discomfort caused by a wire on your braces can be resolved by moving the wire away from the irritated area with a cotton swab or eraser. If the wire will not move, try covering the end of it with a small piece of cotton or a small amount of wax. If the wire is painful, you can cut it with nail clippers or scissors that have been washed and sterilized in alcohol. If you cannot resolve the wire irritation, call our office for an appointment.

Lost separators
Most patients lose a separator during their treatment. Do not worry about losing a separator, but call our office to see if it needs to be replaced.

The forms can be filled out and submitted online. We always keep your safety in mind; therefore, our patient forms are secure to ensure patient confidentiality.

Patient Form Instructions:

  1. The forms can be enlarged to fit your screen using the options provided in Adobe Acrobat Reader. Click on View>Zoom To.. and select the appropriate level of magnification.
  2. For questions with options listed as 'Y' or 'N' indicating 'Yes' or 'No' respectively, click on the letters to make a choice. If Option boxes are provided, please click on them to make a selection.
  3. In places where a signature field is provided, please enter your full name. We may require you to sign the forms again during your first visit to our office.
  4. If you are submitting more than one form, please enable the Auto-Complete feature in Adobe Acrobat Reader to store the information entered in the form fields. This will be helpful when you have to enter repeating information.
